تطلُ حديقة المستشفى على النهر مباشرةً. يرتفعُ سطح أرضيته بمقدار ستة أمتار. سياجٌ حديدي مشبك يسورُ واجهة المستشفى من جهة النهر، يتوسطه مخرج بسيط هو جزء من جسد السور، مغلق دائماً ولا يبدو أنه قد فُتِحَ يوماً ما. يمارسُ عددٌ من المرضى رياضة الهرولة بإشراف أحد مساعدي المعالجين، بينما يجلسُ آخرون ممند لا يقدرون على ممارسة الرياضة يتفرجون على زملائهم بأفواه فاغرة وابتسامات بريئة. باطن المستشفى تمتلئ ردهاته وممراته بالنزلاء والأطباء والمعالجين النفسيين ومساعديهم.
لمْ يختلف صباح هذا اليوم الشتوي المشمس عن أغلب الصباحات إلّا من صياحٍ، يكسرُ سكونه وهدوءه. يأتي هذا الصراخ من المنحدر الذي ينزلق إلى النهر. يتركُ الممرض المشرف على الرياضة الصباحية مرضاه طالباً منهم البقاء في حديقة المستشفى وينظر باتجاه النهر، يتبعه عدد من حرس الأمن الموجودين في المكان .بعد أن تصبح ضفة النهر سهلة الرؤية، يشاهدون “دكتور كريم” يستنجد بهم، وهو يصيح: أَلَمْ أطلب منكم إحكام قفل المخرج، لإن المريضة سارة تحاول النزول الى النهر والانتحار.لا يكترثون لندائه، يطلبون منه الصعود، ولكن شدّة انحدار الضفة تجعل المهمة شاقة. يعود المسعفون أدراجهم، شاقين طريقهم وسط تجمع عدد من المرضى وأغلب كادر المستشفى، مشكلين سوراً موازياً لسور الألومنيوم المشبك .يطلب مسؤول الأمن توفير سلم للنزول. يجلبون السلم على عجالة، ويضعونه لمساعدة الدكتور كريم بالصعود، لكنه يرفض ذلك ويصّر على نزولهم للبحث عن النزيلة سارة. لمْ تستغرق العملية سوى دقائق معدودة حتى يهبط فريق إلى حيث يوجد الدكتور كريم.
يتعرض الفريق بعد النزول الى توبيخ شديد من قبل الدكتور: ألَمْ أقُل لكم مراراً وتكراراً أن تأخذوا حذركم وتراقبون سارة جيداً، فهي تعاني من اكتئاب شديد ومن أفكار تسلطية تدفعها بقوة للانتحار غرقاً، ولكنكم كالعادة لا تأخذون كلامي على محمل الجد. ينظر بعضهم نحو بعضهم الآخر وكأنهم يلومون أنفسهم . يزداد الدكتور غيضًا وهو يصيح: ها أنتم تصمتون كالعادة، مع كل فقدان وخسارة لنزيل أو نزيلة نتيجةً لإهمالكم وعدم تفانيكم وإخلاصكم في عملكم.
وهو يحاول طمأنة الدكتور على أنهم سيعثرون على سارة، يطلب الدكتور ناجي من الفريق التعامل بحذر مع دكتور كريم، فحالته مزرية جداً وتبدو عليه علامات الانهيار. يستمر الدكتور كريم بالامتعاض: ها أنتم مرة أخرى لا تستطيعون الإجابة! يهدؤون من روعه ويمسكون به برفق ويصعدون أعلى المنحدر. بدا زميله دكتور ناجي متأثراً جداً، لاسيما وهو يرى دموعه تجري على خديه، وعينيه لا تريدان مفارقة النهر.
جميع العاملين في المستشفى يتعاطفون بشدة مع وضع الدكتور كريم، الذي تربطه بالمستشفى علاقة تمتد لسنين طوال، سواءً يوم كان من أبرز أطبائه النفسانيين، أو حينما صار واحداً من نزلائه الدائمين!

The garden of the hospital directly overlooks the river. The surface of its floor rises almost six meters. The frontispiece of the hospital is blocked by an iron fence from the side of the river. There is a simple exit in the middle of the fence which is always closed and it does not seem to be opened at all.
Some of patients are jogging under the supervision of a therapist’s assistant while others who are unable to exercise sit and watch their colleagues with open mouths and innocent smiles. Lobbies and corridors are filled with inmates, psychiatrists, therapist and their assistants.
This sunny winter day does not differ from most mornings but a shout breaks its stillness and calm. This scream comes from the slope that slips to the river. The nurse who’s supervising the morning sport looks towards the river than asks the patients to stay in the garden and leaves followed by some of the security guards at the place.
After they can see the river bank clearly, they see “Dr. Kareem” seeks help from them and shouts: “Did I not ask you to tighten the lock of the exit? The patient Sarah is trying to go down to the river and commit suicide. “They do not care about his call, asking him to ascend, but the steep slope of the bank makes the task arduous.
The paramedics go back, making their way among the gathering of patients and most of the hospital staff, forming a wall, parallel to the aluminum lattice fence. The security officer asks for a ladder to go down. In a hurry, the ladder is brought, to help Dr. Kareem to climb, but he refuses and insists that they go down to search for the inmate, Sarah. The team takes a few minutes to descend to where Dr. Kareem is.
After going down, the team is subjected to a severe reprimand by the doctor: “Did I not tell you over and over again to be careful and watch Sarah well? She suffers from severe depression and authoritarian thoughts that force her to suicide by drowning, but as usual, you do not take my words seriously”. They look at each other as if they blame themselves. The doctor gets angry as he shouts: “You are silent as usual, with every loss of every inmate due to your negligence, lack of dedication and sincerity in your work”.
While Dr. Naji trying to calm Dr. Kareem down by assuring him that they will find Sarah, he asks the team to handle Dr. Kareem with care, as his condition is so dire and he shows signs of a nervous breakdown. He continues to resent: “Here you are again unable to answer”! They calm him down, hold him gently, and go up the slope. His colleague, Dr. Naji, looked quite affected, especially when he sees the tears running down his cheeks, and his eyes keep looking at the river. All hospital staff sympathize greatly with Dr. Kareem’s condition. He has a long relationship with the hospital extend over many years, whether when he was one of its most prominent psychiatrists, or when he has become one of its permanent inmates!
